STAY AND PLAY POLICY

CLUB COMPETITIVE DIVISION OPT-OUT FORM
Club teams that wish for a less competitive division can opt out of the club competitive division prior to USQ-hosted national qualifiers

COACH POLICY: EMERGENCY EXCEPTION
Report any instances where a team's certified coach needed to leave the event site due to a medical or personal emergency that arises during an event

CONCESSION POLICY
Outlines the circumstances under which a team may concede a match and the process for doing so

EQUIPMENT POLICY
Specifies the standards and requirements for all player gear used in official play, including safety guidelines and approved equipment types

EXCEPTIONAL CIRCUMSTANCE: GAMEPLAY REQUIREMENTS EXEMPTION REQUEST
A team must fill out this form if they feel there has been exceptional circumstance(s) that has caused the team to be unable fulfill gameplay requirements for the season

Defines expectations for sportsmanship, respectful behavior, and integrity to maintain a safe and welcoming playing environment

Submit equipment such as concussion headbands, knee braces, etc. for approval via this form

PLAYER NUMBER POLICY
Establishes guidelines for assigning and displaying player jersey numbers in official matches

PLAYER TRANSFER POLICY
Regulates how and when players may switch teams or rosters during the competitive season. The league-wide transfer deadline is Sunday, April 12 at 11:59 pm ET for 2026

USQ NATIONAL QUALIFIER REFREE CREW FORM

All teams attending a USQ National Qualifier are required to submit the team's referee crew

USQ NATIONAL QUALIFIER REGISTRATION

All teams are required to register for qualifiers to collect information and confirm attendance at the event. This registration is due by January 14
